Gynecomastia

What is it?

It is an alteration of the male breast that is characterized by an increase in size, sometimes considerable and with ptosis (drooping), usually bilateral, but which can be asymmetrical. It is characterized by hypertrophy of the fat or mammary gland and often appears in adolescence or young adulthood, causing discomfort and social withdrawal.

In most cases there is no known cause (idiopathic) but sometimes it is associated with obesity, physiological endocrine changes or underlying diseases.

It is indicated for men who have aesthetic concerns about the volume and projection of their breasts, with an impact on their choice of clothing or which leads to interpersonal and social isolation and withdrawal, especially in activities that involve exposure, such as the beach, gyms, etc. It is indicated for men who do not have underlying medical conditions:
  • Healthy
  • Men whose breast development has stabilized or, if adolescents, who are informed and aware of the possible possibility of additional treatment after growth has stabilized
  • Non-smokers
  • With expectations appropriate to your body situation and intrinsic conditions, such as skin elasticity and laxity.

Depending on the underlying cause of breast hypertrophy (excess fat or hypertrophy of the mammary gland) and the excess skin/size of the areola, the surgical treatment of gynecomastia can be performed through liposuction (link) or surgical reduction (subcutaneous mastectomy).

If gynecomastia is caused essentially by an increase in fat (it can also occur in the context of obesity), it can be treated with liposuction, with minimal incisions.

In cases where there is glandular hypertrophy, or there is excess skin or areola that warrants excision, breast reduction surgery is performed through a hemi-areolar or peri-areolar incision, with surgical removal of the tissues, reduction in the size of the areola and correction of the ptosis; it can be combined with liposuction.

Breast reduction is performed under general anesthesia, often with a laryngeal mask, a milder type of general anesthesia that allows post-operative recovery to be faster and better tolerated.

The location and extent of the scars varies depending on the surgical technique used.

When liposuction is performed, the incision is minimal, usually less than 5mm, located in an area of the lower chest wall that aesthetically results in a barely perceptible scar.

When the choice falls on the surgical reduction of a simple subcutaneous mastectomy, the scar is hemiareolar, camouflaged at the transition between the areola and the surrounding skin.

If there is also areolar reduction and excess skin, the scar is located around the entire circumference of the areola, also camouflaged at the transition between the areola and the surrounding skin

In an outpatient setting or with a short hospital stay of less than 24 hours, recovery can normally take place at home. You should wear a compression bandage for the next 3-4 weeks, comply with the medication and all the other recommendations of the medical team. You will be advised to limit activities involving arm movements for the first few weeks, but not to limit daily personal care and routine activities. You should avoid intense exercise and exertion for the first few days.
